I offer virtual one-on-one nutrition education and counseling for adolescents and adults (age 16+) in Washington State, Texas, Illinois, and Connecticut.

Many states do not require dietitian licensing; thus, I am also able to offer virtual nutrition services in Arizona, California, Colorado, New Jersey, and Virginia.

If you are planning on using insurance to pay for our time together, please contact your insurance company before your initial session and ask the following questions:

  • Is Sammantha Paulsen an in-network provider with my insurance plan? (NPI number: 1558974766)

  • Do I have insurance coverage for outpatient nutrition counseling? (CPT codes: 97802, 97803; diagnosis code: Z71.3)

  • Does my plan have restricted diagnoses? If so, what are they?

  • Is a referral or prior authorization required?

  • Do I have to meet a deductible before my insurance will cover nutrition sessions?

  • Do I have to a co-pay or co-insurance? If so, how much per visit?

  • How many sessions are covered?

  • Are telehealth services covered?

For all other insurance plans - Many insurance companies may reimburse for some or most of the cost of out-of-network nutrition services. If I am out-of-network with your insurance provider, a superbill can be provided at your request for you to seek possible reimbursement from insurance. Please note that a superbill does not guarantee insurance reimbursement.
